Case Study #1

Early diagnosis and management of a chronic medical condition.

Age: 4

Prompt:
Therapists noticed a change in the client's energy levels and behavior at the clinic.
Intervention:
Therapist recommended a follow-up with the child’s PCP. Screening blood work revealed a diabetes diagnosis. Springtide coordinated with PCP to change feeding behaviors to diversify diet and ensure adherence to medications. Therapist and SLP worked together using naturalistic teaching to improve clients communication relative to diabetic symptoms.
Outcome:
Early diagnosis led to better control of diabetes and avoidance of further hospitalization. Client can now express feelings associated with blood sugar levels and demonstrates preferences for healthy foods at the grocery store.

Case Study #2

Reducing the need for surgical intervention.

Age: 5

Prompt:
Client has a dual diagnosis of cerebral palsy and autism spectrum disorder and has required surgeries to address muscular tension.
Intervention:
The behavioral therapist, in partnership with the OT, focused on building up the clients upper body and core strength, allowing them to participate in functional activities independently, per their desire.
Outcome:
The client has gained enough strength to propel forward on her wheelchair with one hand, potentially reducing the need for corrective surgeries and increasing their independence.

Case Study #3

Impact on ED and Inpatient Utilization, Family Support.

Age: 17

Prompt:
Prior to joining Springtide, the client experienced frequent ED visits, resulting in long inpatient psychiatric stays.
Intervention:
During a behavioral crisis, parent called Springtide instead of going to the ED. Clinician was able to de-escalate the situation over the phone and follow up in-clinic the next day.
Outcome:
No ED visits or inpatient stays since the patient has joined Springtide!
